Schedule as a Guest or Through a MyChart Account

Guest

1. Choose “Schedule as Guest”
2. Choose “Family Medicine”
3. Choose “Skin Screening”
4. Choose an available appointment time
5. Add an appointment note (something you'd like the doctor to know) or type n/a
6. Choose “Schedule as Guest”
7. Complete patient details for the person receiving services (use forward slashes in DOB), then choose “Next”
8. Complete insurance information or choose "No insurance" from the dropdown, then choose the payer of uncovered costs (REMEMBER: These appointments are FREE. Neither you nor your insurance will be billed.)
9. Choose "Submit"
10. Be sure to add your appointment to your calendar.

*You can schedule an appointment by logging into your MyChart first, but we recommend starting with "Schedule as Guest" then logging in at Step 6.

1. Choose “Schedule as Guest”
2. Choose “Family Medicine”
3. Choose “Skin Screening”
4. Choose an available appointment time
5. Add an appointment note (something you'd like the doctor to know) or type n/a
6. Choose “Log in and schedule”
7. Enter username and password, then choose "Sign in"
8. Choose the person you're scheduling the appointment for, then choose "Continue"
9. Be sure to add your appointment to your calendar.
