Every three years, Bayhealth does a Community Health Needs Assessment survey to help us better meet your needs. Your voice matters, and we invite you to take our 2025 survey. By participating, you provide valuable insights that shape the programs, services and resources we develop to improve overall community well-being. Your voice plays a crucial role in ensuring we address real health needs and make a meaningful impact where it’s needed most.
The survey is available in English, Spanish and Haitian Creole. It takes about 5-7 minutes to complete. You can take it on your cell phone, computer or other mobile device.
At the end of the survey, enter your name and phone number for a chance to win one of five $100 gift cards as our way of saying thank you for taking the survey. Surveys must be completed by April 30, 2025 to be eligible for prizes. Exclusions apply. See website for details.
